Mon, Thurs, Fri (week 1), Tues, Fri (week 2)GENERAL DESCRIPTION SUMMARYThe Home Care RN Case Manager provides, coordinates, teaches and supervises the nursing care of Hospice patients and families/caregiversWorking in collaboration with other members of the interdisciplinary team/group, the RN Case Manager plans and manages the nursing component of the hospice caseloadThe RN Case Manager makes initial and ongoing nursing assessments, judgments and treatment decisions based on patient and family/caregiver needs and wishes and in accordance with her/his professional skills.
Experience RequirementsMust have one year of staff nurse experience in a comparable setting/unitHospice and/or palliative care experience is strongly preferred.
Education RequirementsMust have a degree in Nursing from an accredited nursing programA BSN degree is preferred.
Required Certificates and/or LicensesMust have licensure from the governing body for practice for the District of Columbia, State of Maryland, and/or the Commonwealth of Virginia, based on the state/jurisdiction of practice, or licensure from compact stateHealthcare Provider BLS (CPR) is required to practice in the State of Maryland and the District of ColumbiaA CHPN certification is a plus.
