Job Description The Director, Clinical Programs, will work directly with national and market teams, including Care Management, Population Health, Data Analytics, and Care Centers to ensure the development and growth and success of Privia's various clinical programs. They will be responsible for a wide-variety of areas, including the operations and provider engagement within existing programs, strategy and growth of clinical programs, and development and incubation of new clinical programs. These programs include the care of at risk patients using care management, remote monitoring, performance improvement tools (people, process and technology). Examples of current or planned programs are Hypertension management, Diabetes, Behavioral Health, Advanced Illness Management, Clinical Research, and Home Care. They will be ultimately responsible for achieving quality, cost and patient experience goals under our value-based contracts. Primary Job Duties:
Collaborate with the Chief Clinical Officer, SVP Value-Based Operations, and other team leaders to expand Privia's Clinical Program strategy. Leverage data to understand our patient populations across key value-based contracts, prioritize areas of opportunity and impact, and measure success of Clinical Programs. Translate vision and strategy into an operational plan, then execute on that plan. Prioritize initiatives based on reimbursement to value-based contracts, and the sickest patients. Integrate the approach of key Privia teams, including Complex Care, Chronic Care, and Transitions in Care, Behavioral Health, and Medication Management into Clinical programs. Aggregates, reviews and analyzes operational metrics and outcomes data of designated programs. Utilizes data, industry knowledge to develop plans to increase operational efficiency and engagement with current programs. Maintains ownership over designated Clinical Programs responsibilities including but not limited to distribution of revenue and coordination with the Finance team, performance against key metrics, patient and provider engagement, and vendor relationships. Discusses data and actions with market teams & providers and care center staff to create accountability and engagement. Manage the team to key metrics, demonstrating a connection between the team's activities to outcomes, such as total cost of care and quality. Collaborate with the Product & Technology team to develop Privia's proprietary patient engagement and care management platforms. Must comply with HIPAA rules and regulations. Minimum
Qualifications:
RN, NP, PA, MD, or DO At least 5 years of experience in a Clinical management role Experience in risk-bearing provider and/or health plan organizations Experience developing clinical programs Familiarity with reimbursement models such as pay-for-performance, shared risk, and full capitation Credible with a variety of clinical team members - nurses, pharmacists, doctors, etc. Highly collaborative across an organization with multiple stakeholders Excellent at finding and developing new talent Ability to prioritize based of the needs of the organization Analytical and comfortable with numbers - will use ROI analyses to drive decisions Must comply with HIPAA rules and regulations Physical Demands:

Salary Range:
$150K -- $200K
Minimum Qualification
