Who You'll Be Working With:
The Director of CPP reports to the Vice President of CPP. The Director will support all CPP activities in developing a comprehensive strategy and operating infrastructure aimed at maximizing performance across the clinical markets, redesigning clinical programs, workflows, improve efficiency, and deliver the best patient care possible within our partner markets.
The Director will build out and support implementation of CPP offerings in a highly visible internal stakeholder-facing role. The Director will serve as the subject matter expert in clinical operations, clinical IT, clinical quality, and clinical performance improvement for a range of clinically focused initiatives. The Director will be responsible for determining courses of action in complex situations that may not be addressed by existing policies, procedures, or protocols. The Director will be aid in the establishment of organizational objectives for existing markets.
The Director will be responsible for supporting the Vice President in leading the development, implementation, and measurement of effectiveness of clinical programs and performance offerings across all markets. The Director will also work closely with the Value-Based EMR Integration, Transformation, and Analytics teams, among others.
What You'll Be Doing:
Serve as lead project manager for market engagements, including coordinating complex work streams and cross-functional teams
Support the analysis, review and consultation of CPP efforts including gaps in care, workflow enhancements, clinical data, clinical analytics, and staffing
Assist with the development of clinical operational workflows and best practices
Assist with the education about stratification, program descriptions, NCQA, etc.
Work with internal stakeholders to define ways to measure and report CPP activities
Identify tools and resources available to enhance practice optimization capabilities
Responsible for identification and interpretation of market trends, developments, and marketplace factors impacting physician success in value-based care
Support internal and external CPP marketing strategies and business development opportunities
Partner Relations and Business Development:
Responsible for working with the internal Senior and Managing Directors of Clinical Operations, Market Presidents, Market Medical Directors and physicians to provide a full spectrum of consulting services by facilitating and translating strategic direction into meaningful implementation plans
Collaborate with external and internal clinical leadership to execute on market-specific strategies
Lead and support business development opportunities (e.g., creation of sales presentations, responding to RFPs, writing SOWs, etc.), specifically related to enhancing PO business and sales
Performance Management:
Collaborate with primary care providers to optimize care coordination, care gap closure, coding and documentation through workflow redesign and optimization
Support analytics in development of operational and cost performance reports designed to track progress, engage primary care providers in performance improvement and track ROI
Support the development of practice educational programs and engagement strategies
Physician and Practice Relations:
Provide leadership support to strengthen relationships between physicians and leadership as well as physicians and the value-based business centralized organization
Provide assistance and develop relations with physicians and practice management in the market to solve operational challenges and build trust within the markets
The Experience You Need (Required):
5 to 7 years of experience in a Healthcare space
Consulting skills from assessment through implementation
Experience with implementation of
Knowledge of risk adjustment, HEDIS, STARS, payer partnerships, ACOs and practice workflows
Practical experience in physician practice settings
Excellent presentation and facilitation skills
Excellent written and verbal communication skills and the ability to communicate/collaborate effectively with all levels of employees, management, executives, providers, and external organizations
Ability to translate complex clinical and operational concepts into a cohesive, visual story
Strong history of quickly building relationships, gaining credibility, influencing and partnering with business leaders and peers across the organization.
Excellent proficiency in Microsoft Office Suite or similar products
Willingness to travel 50 ? 75%
Finishing Touches (Preferred):
Bachelor's prepared Registered Nurse
Masters level degree

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
