Hospital Extensivist (RN)

Overview of the Role:
We are looking for a patient-centered, entrepreneurial, and organized individual to pioneer our Hospital Extensivist program. The extensivist will be responsible for ensuring appropriate transitions in care for each patient seen in the emergency room. In collaboration with the ED practitioner and the patient's primary community provider the patient will be safely and appropriately discharged from the ED with necessary support and follow-up in the community. Working as an integral member of the Population Health Team at Privia Health, the successful candidate will work closely with Privia Medical Group providers and Medical Directors to create an effective program and provide a best-in-class patient experience.
Primary Responsibilities
Respond to real-time Emergency Room ADT notifications by contacting designated hospital emergency room personnel to collaborate and prevent potentially avoidable hospital admissions.
From a hospital based, or other in-market location, travel as necessary to one of a couple local Emergency Departments to collaborate with ED personnel and family members in support of the plan of care.
Have and develop local delivery system knowledge, expertise, and relationships in order to be able to rapidly develop and execute a community-based care plan.
Communicate real-time with the patient's Privia Medical Group provider who is expected to assume responsibility for the patient to develop the plan of care and provide direction about appropriate follow-up.
Communicate real-time as necessary with a back-up Privia Medical Group Medical Director whose role is to support the extensivist in (i) identifying the PMG provider accepting responsibility for the patient's care, (ii) developing and executing the proper plan of care, (iii) collaborating with the hospital and emergency room personnel.
Evaluate the need for outpatient services and as appropriate, assess, analyze, plan, implement, coordinate, monitor and evaluate options and services to meet individuals health needs
Anticipates care needs along the continuum of inpatient and outpatient services and facilitates coordination across the network with providers, patients, and caregivers to assure timely transfer to the appropriate Level of Care
Act as a liaison and patient advocate between patient, patient families, payers, hospital attending, and Privia PCP by providing supporting medical information and facilitating communication through the continuum of care
Help support the coordination efforts with services related to ED discharge
Works effectively with Nurse Care Manager to coordinate case transition as appropriate
Identify and build effective relationships with Hospital staff, Privia providers, and a broad network of community resources
Document clinical information, discharge plans, level of care changes in designated area
Evaluate the effectiveness, necessity, and efficiency of the discharge plan of care and communicate with the PCP / Medical Director and Care Team to recommend changes
Minimum Qualifications
RN, BSN with Case Management, Critical Care experience preferred
Inpatient experience with working clinical knowledge expertise
Knowledge or experience using Care Transitions Intervention
Firm understanding of medical terminology, disease pathways, and procedures

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
